Lesion Sterilization and Tissue Repair in Endodontics

ABSTRACT Lesion Sterilization and Tissue Repair (LSTR) represents a paradigm shift in contemporary endodontics by emphasizing biological disinfection over conventional mechanical instrumentation. The technique was originally introduced to manage infected primary teeth where complex canal anatomy, physiological root resorption, and limited patient cooperation restrict the effectiveness of traditional endodontic procedures. LSTR employs a combination of antibacterial agents, commonly referred to as Triple Antibiotic Paste, delivered through a specialized vehicle system to eliminate pathogenic microorganisms from dentinal, pulpal, and periradicular tissues. This review highlights the microbiological and pharmacological basis of LSTR, its clinical protocol, and its applications in both primary and permanent dentition. Additionally, recent modifications, regenerative endodontic implications, advantages, limitations, and ethical considerations related to antibiotic usage are discussed. The growing evidence supports LSTR as a minimally invasive, cost-effective, and biologically sound alternative in selected endodontic cases, particularly within pediatric and regenerative endodontics.
