im:-To ensure preparation of pre sample collection, proper Sample collection, packaging, transport and storage.To collect appropriate clinical samples as indicated for diagnosis of 2019- nCoV Lab /Hospital shall ensure that the proper sample collection, packaging, transport and storage procedure will be conducted for COVID-19 patients/suspects as per the ICMR guidelines. Request forms contain information regarding the identity of patients and authorized requester. It also has the following:Format of request forms is determined in discussion with the users of the laboratory services and as per the requirements of NABL 112. The primary sample collection, transport and handling procedures are prepared and are available at the site of sample collection area — primary sample collection centre and all the wards.
