Radix Entomolaris and Paramolaris: Review, Case Report in Mandibular Molar

ABSTRACT The knowledge about the external and internal anatomy of tooth is essential for successful dental practice. Success of endodontic treatment depends on the proper identification of all the canals, thorough chemomechanical preparation followed by three-dimensional obturation with hermetic seal. Failure of any of these steps may occur due to unusual tooth morphology. Majority of mandibular molars are two rooted with two mesial and one distal canal[1] but in few teeth, the number of roots and canals vary. When there is variation in the number of roots, If extra root located lingually is called Radix Entomolaris (RE) or located bucally called Radix Paramolaris (RP). Understanding of the presence of an additional root and its root canal anatomy is essential for successful treatment outcome. Anamolies in tooth are often encountered which poses difficulty in dental treatment. However, in such a case, an awareness and understanding of this unusual root and its root canal morphology can contribute to the successful outcome of Root Canal Treatment. The present case report demonstrates an extraordinary anatomical configuration and supplements previous reports of existence of such configuration in mandibular first molar. Additional radiographs taken from different horizontal projections, i.e 20 degree mesial and 20 degree distal reveals the basic information about the anatomy of additional third root . This case report presents Endodontic treatment of two mandibular molars with Radix Entomolaris or paramolaris, the external variation, internal anatomy of Radix entomolaris and Radix paramolaris.
