Short-Term HRV Indices for Screening Cardiac Autonomic Neuropathy in Type 2 Diabetes

ABSTRACT Cardiac autonomic neuropathy (CAN) is an under diagnosed complication of type 2 diabetes mellitus (T2DM) that impairs heart-rate control and blood-pressure regulation. Although Ewing’s cardiovascular reflex tests remain the gold standard, they are time-consuming and resource intensive. Short-term heart-rate variability (HRV) derived from a brief electrocardiogram provides a fast, non-invasive alternative for screening. This study evaluated whether indices from a five-minute HRV segment can indicate early autonomic involvement in T2DM and identified informative metrics across time, frequency, and non-linear domains. Resting ECGs of 10–12 minutes were recorded; artifact-free five-minute windows were processed with a Hamilton-style QRS detector (3–25 Hz band-pass) and strict quality control.
