Systematic Review on the Role of Panchakarma in Plantar Fasciitis

ABSTRACT
Plantar fasciitis is one of the most common musculoskeletal disorders causing heel pain and difficulty in walking, significantly affecting the quality of life. In Ayurveda, the clinical presentation of plantar fasciitis can be correlated with Vatakantaka and is mainly associated with vitiation of Vata Dosha. The causative factors such as excessive walking, prolonged standing, overexertion, improper posture, and trauma lead to aggravation of Vata resulting in pain, stiffness, tenderness, and difficulty in movement. Classical Ayurvedic texts including Sushruta Samhita, Charaka Samhita, Ashtanga Hridaya, and Ashtanga Sangraha emphasize the importance of Panchakarma therapies in the management of Vata Vyadhi. Panchakarma procedures such as Snehana, Swedana, Basti, Upanaha, Agnikarma, and localized therapies help in reducing pain, inflammation, stiffness, and improving mobility by pacifying aggravated Vata Dosha. These therapies also enhance circulation, relieve muscle spasm, and promote tissue healing. The present systematic review aims to evaluate the role of Panchakarma in the management of plantar fasciitis through classical references and contemporary clinical understanding. The review highlights the therapeutic efficacy, safety, and holistic approach of Panchakarma in providing symptomatic relief and improving functional outcomes in plantar fasciitis. Panchakarma, along with proper dietary and lifestyle modifications, offers a promising and cost-effective approach for the management of plantar fasciitis and prevention of recurrence.
